Collect three (3) UPC codes from any package of NESTLÉ® TOLL HOUSE® Morsels or NESTLÉ® TOLL HOUSE® Refrigerated Cookie Dough products. Together in an envelope, mail your UPC codes, dated receipt for each purchase and a 3" x 5" card with your neatly-printed name, address, city, state, ZIP, phone number with area code and e-mail address (optional) to:

Once we receive your information, we'll mail you your free plate via U.S. Mail. The shipping and handling is also free.
